Screen to Live Program

Screening saves lives...
Screen to Live is a pilot, community based education, navigation and quality improvement program in the Englewood neighborhood located on the south side of Chicago, Illinois. It targets uninsured and publicly insured, African American women aged 40 and over for mammogram screenings. The purpose of the program is to educate women through community based events, link them to free, high quality screening services where they live and facilitate free resources for diagnostic exams and treatment as needed. Our annual goal is to educate 400 women and navigate 200 of them to screenings. We have partnered with St. Bernard Hospital, the Chicago Department of Public Health and Mercy Hospital and Medical Center on this initiative. To date, the project has educated over 1,500 women and navigated several hundred of them to free services.
